Most annoying deaths?

I'm wondering what our most frustrating deaths have been in our Minecraft worlds? What's the most you remember losing in one death? Ever felt like a death was total bullshit? Feel free to share. I'll go first - this just happened and inspired me to make this post.
I was playing my a1.2.6 world, navigating a pool of lava in a cave, using gravel to build safe ground. I was shifting over the edge of some stone and when I placed gravel, it dropped, and I dropped with it. I lost 9 diamonds, something like 30 iron ore, ~3 stacks of cobble, ~half a stack of coal, my diamonds tools, and my armour. I have enough material to get myself back on track, but the whole point of that little expedition was to re-stock. Definitely the most costly death on my world so far, and even more annoying because I could probably count the number of deaths in that world on one hand. Normally I'm pretty careful. I don't know how it happened because I was shifting on the stone, but while I've been playing on that world I've noticed that shifting isn't always completely reliable.
