Most annoying deaths?

I'm wondering what our most frustrating deaths have been in our Minecraft worlds? What's the most you remember losing in one death? Ever felt like a death was total bullshit? Feel free to share. I'll go first - this just happened and inspired me to make this post.

I was playing my a1.2.6 world, navigating a pool of lava in a cave, using gravel to build safe ground. I was shifting over the edge of some stone and when I placed gravel, it dropped, and I dropped with it. I lost 9 diamonds, something like 30 iron ore, ~3 stacks of cobble, ~half a stack of coal, my diamonds tools, and my armour. I have enough material to get myself back on track, but the whole point of that little expedition was to re-stock. Definitely the most costly death on my world so far, and even more annoying because I could probably count the number of deaths in that world on one hand. Normally I'm pretty careful. I don't know how it happened because I was shifting on the stone, but while I've been playing on that world I've noticed that shifting isn't always completely reliable.

I have my own death related to shifting, actually. I was in the Nether when I duck walked along a wall over the sea of lava. Instead of hugging the wall, I decided for some reason to walk out along the edge of it. Figured I wouldn't fall as long as I held shift. Ha, nope. I bumped into a block in front of me, which - God knows why - caused my character to slip off the wall and into the soup. Thankfully, I'm a dirty savescummer who makes backups every time I play, so little progress was lost  . Now I know forever not to trust the shift key to prevent my guy from falling, even if my pinky weighs 5 tons. Aside from that, I've never had a death that didn't feel like my own fault and as far as I recall, I've never lost a significant amount of items due to death. Though, in terms of my most annoying deaths, it has to be the literal dozens of times I've lost my grip and fallen to my death trying to build in the sky...
